Abstract

Background: Within the context of pediatric Type 1 Diabetes (PT1D), transition management is the process of shifting PTID responsibility from caregiver to child as they approach adulthood. With PT1D on the rise, effective transition management strategies are increasingly important to ensuring safe self-management of PT1D. The main objective of this systematic review is to summarize developmentally appropriate interventions that support youth and caregivers through the transition of PT1D.

Methods: CINAHL, PubMed, and Web of Science were used to identify studies for this review. Inclusion criteria included primary research articles in English, published from 2014-2024, and focused on children ages 1-19 who were transitioning care of PT1D. Non-English articles were excluded. A Qualtrics survey developed using the Cochrane Handbook for Systematic Reviews was used to assess for risk of bias. Data management research platforms Zotero, Rayyan, and Qualtrics screening surveys were used to assess and synthesize studies meeting inclusion criteria.

Results: Themes in clinician and caregiver transition interventions emerged. 37 studies were included following abstract screening, full-text review, and assessment for risk of bias. One study examined early childhood transition, five studies examined middle childhood, and 31 studies examined transition interventions in later childhood.

Discussion: Appropriate transition interventions tailored to developmental stages can help clinicians and caregivers ensure a successful transition of PT1D care to the patient. Rigorous investigation to validate strategies for different age groups are needed. To support this process and improve T1DM outcomes, research, and recommendations should be further reviewed to assist in tailoring interventions accordingly
